North Dakota Autism Spectrum Disorder Survey
The Governor's Autism Spectrum Disorder Task Force wants to hear from you! It is conducting a survey to help determine what is needed in the way of services for individuals who have a diagnosis on the autism spectrum. It is also an opportunity for agencies and providers to identify the autism spectrum disorder specific services that are already available in North Dakota.
The survey can be completed on a computer. You will need to finish and submit it within two hours of opening the following link: https://eforms.nd.gov/lfserver/SFN59472
NOTE: You need to have the Adobe Reader version 8 or 9 to complete the survey.
You only need to complete the survey once. If you receive more than one invitation, please ignore any duplicate invitations. The survey is available until May 7, 2010. Thank you!
